Acceptable Proofs Of Residency

Two proofs of residency are required for all players using their residence to be eligible to play at the Nationals.  Each one must be dated between 2/1/16-2/1/17and the address must be visible. Please upload these to your player's registration page.   If you are unable to upload them, you may bring copies to the evaluations. Here are some examples:  
 
Driver's license
Voter's registration
School Records
Utility Bills (gas electric, water/sewer, phone, mobile phone, heating, waste disposal)
Insurance documents
State records
 
Don't forget to upload a copy of their birth certificate, also.
 
Those that are using their school to be eligible to play at the Nationals must print a copy of the 2016 School Enrollment Form and have the Principal sign to verify that the player was enrolled before October 1, 2015.  This form can be found on the website under Downloadable/forms.  Please upload this also, with a birth certificate.
